A Long-Awaited Reckoning in America’s Formula Aisles
Bringing home a newborn conjures universal worries—health, safety, the mysteries of sleep. But since the 2022 baby formula shortage rattled shelves and nerves nationwide, one fear has persisted: What, exactly, is really in the canisters and bottles that millions of U.S. babies depend on for nourishment?
This month, the Food and Drug Administration (FDA) and Department of Health and Human Services (HHS) announced a sweeping review of the nutrients required in infant formula—the first such evaluation in over a quarter of a century, and a move long demanded by pediatricians, nutritionists, and anxious parents alike. Codenamed Operation Stork Speed, this initiative seeks to bring formula standards in line with the latest research and to address deeply rooted gaps in transparency and regulation that have haunted the industry for decades.
Roughly 75% of U.S. babies consume formula at some stage, according to the CDC. Yet until now, foundational safeguards have rested on regulations codified back in 1988 and barely revised since 1998. Imagine relying on nutrition science that predates smartphones. Dr. Priya Jaising, a pediatrician at Yale School of Medicine, observes, “There is an urgent need to update these standards, especially given our growing understanding of early nutrition’s impact across a lifetime.”
Cracks in the System: Sweeteners, Contaminants, and Muddled Labels
You might expect the formula on U.S. shelves to mimic breast milk’s nutrition and purity. Reality diverges sharply. The current FDA requirement lists 30 essential nutrients, aiming to provide the mixture of calories from fats, carbohydrates, and proteins that infants need. But the U.S. system allows for high levels of added sugars, and until very recently, contained few strict limits on heavy metals or other worrisome contaminants that have appeared in repeated studies by Consumer Reports and Healthy Babies Bright Futures.
Compare this with the European Union: their formula standards prohibit sucrose and corn syrup solids, and impose stricter thresholds for contaminants like lead and arsenic. As Dr. Jennifer Lowry, chair of the American Academy of Pediatrics’ Council on Environmental Health, points out, “European parents have higher assurances about what isn’t in their formula, not just what is.” American parents have often found themselves navigating confusing ingredient lists and misleading marketing more than science-driven protections.
Beyond that, the FDA now acknowledges compelling gaps concerning the long-term health implications of early formula use. As they launch their review, federal officials are encouraging the public and experts to weigh in—asking whether standards should require new nutrients (think: omega-3s, probiotics), adjust maximum or minimum levels, or ensure clearer, simpler labels that don’t require a nutrition degree to decipher.

Who Benefits—and Who Gets Left Behind?

Families of color, low-income parents, and those reliant on government nutrition programs are disproportionately likely to depend on formula. According to Harvard nutrition researcher Dr. Elsie Taveras, “Any deficiency or contaminant in formula falls hardest on the most vulnerable.”
Historical parallels abound: The U.S. once lagged behind in fortifying foods with folic acid and banning lead in gasoline, only pivoting when evidence—and advocacy—became impossible to ignore. Formula policy is no different. Republican-backed deregulatory efforts, under the banner of cutting red tape for business, have routinely slowed the evolution of food safety standards. This episode is a reminder: The risks of regulatory inaction are rarely shared equally, and the political resistance to reform often comes at a high human cost.
Removing regulatory bottlenecks in the name of the free market sounds admirable—until you remember that the “market” is feeding your child. A progressive approach places public health, not corporate convenience, at the core of decision-making. Safety standards should be “dynamic, not stagnant,” Dr. Jaising insists, following the best science available, evolving alongside growing bodies of research and public health need.
Looking forward, the review’s success will hinge not just on new nutrient targets but on whether authorities can enforce rigorous contaminant testing and honest labeling. If the FDA falls short, it risks perpetuating a two-tiered system—where affluent families import European formulas or leverage expensive alternatives, and everyone else is left to make the best of outdated, unregulated options.
